Which boxes you fill on Korea’s immigration application form depends on what you are applying for. A school-age child fills different lines from a worker changing jobs, and only two of the ten applications need a photo.

The form is Form No. 34 of the Enforcement Rules of the Immigration Act, called the integrated application form (통합신청서). It is printed in Korean and English, and in one place the English line leaves out what the Korean line says. Here is every part of the form, in the order it is printed.

What this guide covers
01 Which version of the form you need
02 Which box to tick
03 What the personal details ask
04 Which lines depend on your application
05 What the consent section means
06 What to leave blank and what to attach

Which version of the form you need

Form verified October 2026 · Enforcement Rules of the Immigration Act, Form No. 34 (revised 12 April 2022)

The form comes in three printed versions. Each one tells you to write your answers in Korean or English, including the two Chinese versions.

Three versions of the integrated application form
Enforcement Rules of the Immigration Act · forms attached to the rules
Form Korean name Printed in
Form No. 34 별지 제34호서식 Korean and English
Form No. 34-2 별지 제34호의2서식 Korean and traditional Chinese
Form No. 34-3 별지 제34호의3서식 Korean and simplified Chinese

Overseas Koreans on F-4 have their own integrated form, listed separately on Hi Korea’s forms page. A change of address filed at a community service center also uses separate forms, Nos. 34-4 to 34-8, under Article 49-3 of the Enforcement Rules.

Form No. 34 is on Hi Korea’s forms page under stay-related forms (체류 관련), as a Hangul (HWP), PDF or Word file. The PDF there is the same 2022 version as the one attached to the law.

Which box to tick

The top of the form lists ten applications and reports. Tick the one you are making. Three of them also ask you to write the status you are applying for (희망 자격).

The ten boxes at the top
Form No. 34 · Korean label, the form’s own English, and what it means
Korean label English on the form What it is
외국인 등록 Foreign resident registration Your first registration after arriving
체류자격 외 활동허가 (희망 자격) Engage in activities not covered by the status of sojourn Permission to do something your status does not cover
등록증 재발급 Reissuance of registration card A replacement card
근무처 변경·추가허가 / 신고 Change or addition of workplace A new or an extra workplace
체류기간 연장허가 Extension of sojourn period More time on your current status
재입국허가 (단수, 복수) Reentry permit (single, multiple) Leaving Korea and returning on the same status
체류자격 변경허가 (희망 자격) Change of status of sojourn Moving to a different status
체류지 변경신고 Alteration of residence Reporting a new address
체류자격 부여 (희망 자격) Granting status of sojourn A first status for a person who has none, such as a child born in Korea
등록사항 변경신고 Change of information on registration Reporting a change in registered details, such as a new passport

Sojourn is the form’s word for stay. A grant of status covers two main cases (Immigration Act, Article 23). One is a foreign child born in Korea, within 90 days of birth. The other is a person in Korea who loses Korean nationality, within 60 days. Fees differ by box, and a change of address costs nothing. The guide to immigration fees lists each one.

What the personal details ask

The next block is the same for every application. Write it as it appears on your passport and card.

Personal details
Form No. 34
Korean label English on the form What to write
성명 · 성 / 명 Name in full · Surname / Given names Your family name in the first part, your given names in the second
생년월일 Date of birth Year, month and day, in that order
성별 Sex Tick 남 (M) or 여 (F)
외국인등록번호 Foreign Resident Registration No. The number on your card. On a first registration you do not have one yet.
여권번호 · 발급일자 · 유효기간 Passport No. · Issue date · Expiry date From your current passport
대한민국 내 주소 Address in Korea Your address of stay in Korea
본국 주소 Address in home country Your address in your own country

Your address in Korea matters beyond this form. It decides which office handles your application, as explained in the guide to finding your immigration office.

Which lines depend on your application

The middle of the form has lines that apply to some applicants and not others. These are the ones that need the most care.

Lines that only some applicants fill
✓ School status (재학 여부) — not in school, elementary, middle or high school, with the school’s name and phone. School type is accredited by the education office (교육청 인가), not accredited (교육청 비인가), or an alternative school (대안학교).
✓ Workplace (근무처) — your current workplace and your new one, each with its business registration number (사업자등록번호) and phone.
✓ Annual income (연 소득금액) — written in units of 10,000 KRW (만원). An income of 30,000,000 KRW is written as 3000.
✓ Occupation (직업), intended period of reentry (재입국 신청 기간) and e-mail
✓ Refund bank account (반환용 계좌번호) — only for a first registration and a replacement card, according to the Korean line

The refund account is where the two languages part. The Korean line says to fill it in for a first registration and for a replacement card. The English line names only the first registration. If you are replacing a card, follow the Korean line.

The income line uses the Korean unit 만원, which is 10,000 KRW. Writing the full amount in won makes the figure ten thousand times too large.

What the consent section means

The documents line points to Annex 5-2 of the Enforcement Rules, which lists the attachments for each status of stay and each type of application. Below it is a consent statement under Article 36 of the Electronic Government Act (전자정부법).

Signing the consent lets the officer check some of the required documents in shared government records, instead of asking you for paper copies. If you do not agree, you must submit those documents yourself.

The section has three signature lines: the applicant, the applicant’s spouse, and the applicant’s father or mother (부 또는 모). Sign your own line. If you are unsure whether a family member also needs to sign, ask when you apply.

What to leave blank and what to attach

The last details on the form
✓ For Official Use Only (공용란) — the block at the bottom is for the office. Leave it empty.
✓ Photo — 35 × 45 mm, taken within the last 6 months. The form asks for it only for a first registration and a replacement card.
✓ Revenue stamp (수입인지) — the form says to attach it on the back. The card fee is the exception, paid in cash under Article 73 of the Enforcement Rules.
✓ Date and signature (서명 또는 인) — sign, or use a seal. Either is accepted on the form.

A first registration is one of the two applications that need a photo, and its fee is paid in cash. It is also the application most new arrivals fill in first.
