Health Certificate in Korea: Who Needs One and How to Get It

Square graphic with the text Health Certificate and Korea on a black background

If a manager tells you to bring a 보건증 before your first shift, that word will not match the paper you are handed. The government renamed it. Three official sites — Gov24, the public health portal e-보건소, and a city health center’s service list — print the same document under two names: the health examination result (건강진단 결과서) and, beside it, the older name 보건증. Same paper, two names.

The rules behind it changed in December 2023. The test now covers three diseases instead of two, the result carries a fixed one-year validity, and renewal has to happen within 30 days on either side of the expiry date. Many English guides still describe the older version. This guide covers who has to take the test, when the law says to take it, what it checks, and why the online reissue route fails for a lot of people.

Who needs a health certificate

Rules verified September 2026 · Food Sanitation Act Article 40 · Enforcement Rule Articles 49 and 50

Article 49 of the Enforcement Rule sets the group by the work, not by the job title. It covers operators and employees who work directly on food or food additives: gathering, manufacturing, processing, cooking, storing, transporting or selling it. A kitchen hand, a barista, a packer on a factory line and the shop owner are all in the same group. The article then carves out one exception, worth reading before you book a test.

Who the rule covers
Enforcement Rule of the Food Sanitation Act, Article 49 (1)
Status Who
Covered Operators and employees working directly on food or food additives: gathering, manufacturing, processing, cooking, storing, transporting or selling
Not covered People who only transport or sell fully packaged food or food additives
Not covered Work on chemically synthesized products, or on sterilizers and disinfectants for utensils

The packaged-goods line decides a lot of cases. Ringing up sealed snacks is not the same work as scooping ice cream. If your shift includes any handling of unpackaged food, you are in the covered group.

The duty runs both ways. Article 40 (3) of the Act says an operator may not put a person to work without the examination, so your employer needs the certificate on file for you.

When the law says to take the test

Article 49 (2) is direct about the timing: take the examination before you start the business or start working in it. Not during your first week, and not when someone asks for the paper.

After that first test, the rules set a repeating cycle, and renewal has its own window.

· Once a year. Operators and employees take the examination every year.
· Valid one year. The new term is counted from the day after the previous term expires.
· A 30-day window on each side. Take the examination within 30 days before or after the expiry date.
· One extension. For a natural disaster, accident or illness, the authority may extend the deadline once, by up to one month.

The window is easy to work out. If your certificate expires on 10 April, you can take the next examination any time from 11 March to 10 May. The one-year validity and the 30-day window were both added in the December 2023 amendment.

Plan backwards from your first day
The rule says before you start working, and Gov24 puts the wait for a result at about five days. Booking the test the day before a shift leaves no room for either.

What the test checks

Three diseases, and nothing else. This is a short screening, not a full medical examination, and the list is fixed by ministerial rule, not by the clinic.

The three screening items
Health Examination Rule for Food Service Workers, Article 2 (1)
Item Korean Note
Typhoid fever 장티푸스 —
Paratyphoid fever 파라티푸스 Added December 2023
Pulmonary tuberculosis 폐결핵 —

A guide listing only two items predates the amendment. The Act adds one rule on top of the screening: a person found to have a disease that could put others at risk may not work in the business. Article 50 of the Enforcement Rule holds that list, linked at the end of this guide.

Where to take it and what it costs

Article 3 of the Health Examination Rule names the places: a public health center (보건소), or a general hospital, hospital or clinic. Whichever one you use issues the result form.

A visiting option also exists, but the business operator requests it, not an individual worker: a medical professional comes to the premises and examines staff there.

On price, the rule sets no national figure. Article 5 says a person examined at a public health center pays the fee set by the ordinance of the local government that runs it, so the cost differs by city. The health center serving your address can quote it, and private hospitals set their own charges.

How long it takes and how to pick it up

Gov24 describes the process in two steps: you visit a public health center, fill in the application form and show identification, and the examination is carried out. The service page puts the wait until the result is decided at about five days. Three pickup routes follow, and they are not equally available to everyone.

Three ways to receive the certificate
Gov24 service page · 건강진단결과서(보건증) 발급
Route What you need
In person Return to the same health center where you applied, and show identification
Someone else collects it A letter of proxy for collection (건강진단결과서 대리수령 위임장), plus identification for both people, original or copy
Online e-보건소 (www.e-health.go.kr) → 온라인 민원서비스 → 제증명발급 → 건강진단결과서(보건증). Gov24 also issues it

The proxy route is the quiet useful one. If your shifts overlap with the health center’s opening hours, someone else can collect the paper with the signed form and identification for both of you.

Why the online route may not work for you

The online service carries conditions that are easy to miss until the screen refuses you. The public health portal lists them before the login step.

Four conditions on online issue
1. The result has to be a normal finding. Anything else has to be collected at the health center in person.
2. Only public health institutions are in the system. A result from a private hospital cannot be printed here.
3. The record has to have been filed as a certificate application at the health center’s civil affairs desk, from 2021 onward. A record filed under the tuberculosis program is excluded.
4. Identity verification runs on a simple certificate (간편인증) or a joint certificate (공동인증서). Simple verification needs mobile phone identity confirmation in your own name.

The fourth condition has nothing to do with the examination itself. It is a Korean digital identity problem: a phone contract in your own name, or a certificate from a Korean bank. The portal does process passport numbers and alien registration numbers under Article 24 of the Enforcement Decree of the Regional Public Health Act, so the system is built to identify foreign residents. The obstacle is the login, not the record.

The in-person and proxy routes carry no such requirement. For the online service the portal gives 1566-3232, weekdays 09:00 to 18:00, ARS menu 5 then 1.

What to say at the counter

A health center counter runs in Korean. Show the Korean line on your phone, or read the romanized line aloud.

Frequently asked questions

How to get a health certificate in Korea?
Go to a public health center, fill in the application form and show identification, and take the examination. Gov24 puts the wait at about five days. Then collect the paper in person, send someone with a proxy form, or print it online if your case meets the four conditions above.
How much does a medical checkup cost for foreigners in Korea?
The rule sets no national price. At a public health center you pay the fee written into the local government’s ordinance, so the amount differs by city. Ask the health center that serves your address. A general health screening is a separate scheme with separate rules.
How much is a TB test in Korea?
Pulmonary tuberculosis is one of the three items inside this examination, so it sits under that single ordinance fee rather than being priced on its own. A tuberculosis test taken for another purpose, such as a visa application, is a different procedure with a different price.
Can I take the test at a private hospital instead?
Yes. The rule names general hospitals, hospitals and clinics alongside public health centers, and whichever one examines you issues the result form. One trade-off: the portal holds records only from public health institutions, so a private hospital result cannot be printed online later.
When does the one-year period start?
The day after the previous term expires, not the day you take the test. So taking the examination early inside the 30-day window does not shorten your next term.
