If a manager tells you to bring a 보건증 before your first shift, that word will not match the paper you are handed. The government renamed it. Three official sites — Gov24, the public health portal e-보건소, and a city health center’s service list — print the same document under two names: the health examination result (건강진단 결과서) and, beside it, the older name 보건증. Same paper, two names.
The rules behind it changed in December 2023. The test now covers three diseases instead of two, the result carries a fixed one-year validity, and renewal has to happen within 30 days on either side of the expiry date. Many English guides still describe the older version. Who needs a health certificate

Article 49 of the Enforcement Rule sets the group by the work, not by the job title. It covers operators and employees who work directly on food or food additives: gathering, manufacturing, processing, cooking, storing, transporting or selling it. A kitchen hand, a barista, a packer on a factory line and the shop owner are all in the same group. The article then carves out one exception, worth reading before you book a test.

The packaged-goods line decides a lot of cases. Ringing up sealed snacks is not the same work as scooping ice cream. If your shift includes any handling of unpackaged food, you are in the covered group.
The duty runs both ways. Article 40 (3) of the Act says an operator may not put a person to work without the examination, so your employer needs the certificate on file for you.
When the law says to take the test
Article 49 (2) is direct about the timing: take the examination before you start the business or start working in it. Not during your first week, and not when someone asks for the paper.
After that first test, the rules set a repeating cycle, and renewal has its own window.
The window is easy to work out. If your certificate expires on 10 April, you can take the next examination any time from 11 March to 10 May. The one-year validity and the 30-day window were both added in the December 2023 amendment.
What the test checks
Three diseases, and nothing else. This is a short screening, not a full medical examination, and the list is fixed by ministerial rule, not by the clinic.

A guide listing only two items predates the amendment. The Act adds one rule on top of the screening: a person found to have a disease that could put others at risk may not work in the business. Article 50 of the Enforcement Rule holds that list, linked at the end of this guide.
Where to take it and what it costs
Article 3 of the Health Examination Rule names the places: a public health center (보건소), or a general hospital, hospital or clinic. Whichever one you use issues the result form.
A visiting option also exists, but the business operator requests it, not an individual worker: a medical professional comes to the premises and examines staff there.
On price, the rule sets no national figure. Article 5 says a person examined at a public health center pays the fee set by the ordinance of the local government that runs it, so the cost differs by city. The health center serving your address can quote it, and private hospitals set their own charges.
How long it takes and how to pick it up
Gov24 describes the process in two steps: you visit a public health center, fill in the application form and show identification, and the examination is carried out. The service page puts the wait until the result is decided at about five days. Three pickup routes follow, and they are not equally available to everyone.

The proxy route is the quiet useful one. If your shifts overlap with the health center’s opening hours, someone else can collect the paper with the signed form and identification for both of you.
Why the online route may not work for you
The online service carries conditions that are easy to miss until the screen refuses you. The public health portal lists them before the login step.
The fourth condition has nothing to do with the examination itself. It is a Korean digital identity problem: a phone contract in your own name, or a certificate from a Korean bank. The portal does process passport numbers and alien registration numbers under Article 24 of the Enforcement Decree of the Regional Public Health Act, so the system is built to identify foreign residents. The obstacle is the login, not the record.
The in-person and proxy routes carry no such requirement. For the online service the portal gives 1566-3232, weekdays 09:00 to 18:00, ARS menu 5 then 1.
